Employees
Employees and their dependents enrolled in the UM System’s Custom Network Plan, PPO Plan, and Healthy Savings Plan can obtain diabetic supplies through the medical insurance plan or the prescription drug plan.
Medical insurance plan
- Custom Network Plan and PPO Plan - Plan members may obtain the supplies noted below through an in-network durable medical equipment (DME) provider through their medical insurance at no cost to them (free).
- Healthy Savings Plan - Plan members may obtain the supplies noted below through an in-network DME provider, subject to the applicable plan deductible and co-insurance.

Prescription drug plan
Your prescription drug plan covers the following supplies at the normal costs for prescription drugs (i.e., normal copays/coinsurance apply):
- May offer free glucose monitors on a periodic basis
- Supplies (including diabetic strips, lancing devices, lancets, needles, syringes, and alcohol wipes)
- Generic supplies are available for the Ascensia glucose monitors
- Brand-name supplies are available for One Touch glucose monitors
Please be aware that if you go to a pharmacy for your supplies, the pharmacy will process the purchase under your prescription plan because that is what they are set up to do. Furthermore, if you obtain your diabetic supplies from a retail facility, they will be subject to the annual deductible for the PPO Plan.
Retirees
Retirees and/or their dependents enrolled in a UM System medical insurance plan can obtain diabetic supplies through the plan.
Enrolled in the Healthy Savings Plan or Retiree Health PPO Plan
- Retiree Health PPO Plan - Plan members may obtain glucose monitors, insulin pumps, and supplies from an in-network DME provider through their medical insurance at no cost to them (free).
- Healthy Savings Plan - Plan members may obtain glucose monitors, insulin pumps, and supplies from an in-network DME provider, subject to the applicable plan deductible and co-insurance.
National DME providers are the same as those listed under Employees above (i.e., EdgePark, Arriva, Bryam, etc).
Enrolled in a Medicare Advantage plan
Retirees and/or their dependents enrolled in the Medicare Advantage Base Plan or the Medicare Advantage Enhanced Plan may obtain the following brands of blood glucose testing strips and meters at a $0 co-pay:

- ACCU_CHEK® Aviva Plus with ACCU-CHEK® Aviva Plus test strips
- OneTouch Ultra® 2 System with OneTouch Ultra® test strips

- OneTouch Verio® with OneTouch Verio® test strips
- OneTouch Verio® Sync with OneTouch Verio® test strips
- OneTouch Verio® IQ Sync with OneTouch Verio® test strips
- OneTouch Verio FlexTM System Kit with OneTouch Verio® test strips
In addition to the above brands of test strips and meters, supplies include any brand of lancets, lancing device, glucose control solution (to test the accuracy of the meter), and replacement batteries for your meter.
Medicare Advantage members can obtain diabetic supplies from any pharmacy or DME provider that accepts Medicare. Members simply submit their UHC ID card to be processed under the medical insurance plan.
