Series Description:

This records series applies to electronic documentation related to tests, treatments, and hospitalizations of all patients treated or for services performed at University Hospital and Clinics departments for all patients (adults, minors, students and deceased patients).  This retention also applies to the Schools of Psychology, Orthodontics, Ophthalmology, Dentistry, Vision, etc.

Paper copies of records that have been digitized may be destroyed when no longer of administrative value.

Some electronic record systems are not currently able to destroy electronic records because of system capabilities.  Those records will be considered permanent until the technology allows for automatic deletion at the end of the retention period.
