1. PATIENT TREATMENT RECORDS

These records consist of various documentation related to tests and treatments performed by all University Hospital and Clinics departments for/on patients. Summaries of these tests and treatments are normally in the Patient Medical chart in Medical Records.  This may also apply to the University’s Schools of Psychology, Orthodontics, Ophthalmology, etc. 

Authorized Retention:

  1. Adult Patients (eighteen (18) years old and older):

    Paper Records: Retain for ten (10) years after the year of last treatment, test or visit, then destroy.
    Microfilm/Microfiche Records:  Retain for twenty-three (23) years after the year of treatment, then destroy.

  2. Minor Patients (younger than 18 years old):

    Retain for twenty-three (23) years after the year of last treatment, test or visit, then destroy.
    Microfilm/Microfiche Records:  Retain for twenty-three (23) years after the year of treatment, then destroy.

NOTE: This retention only applies to paper or microfilmed medical records charts.  Electronic records stored in Cerner are to be kept permanently until they have the ability to delete records from their electronic patient chart system.  The prior authorization retained the records for 10 or 23 years from the year of treatment.

2. QUALITY ASSURANCE/CONTROL RECORDS

These records consist of various documentation maintained by hospital departments to verify and monitor the quality of their services, equipment, and other activities of their departments.

Authorized Retention:
Retain for five (5) years, and then destroy.
